Medical Records Request

To access or receive copies of your medical record or to authorize HealthCARE Express to release your medical information to another facility or individual, you must submit your request with a signed authorization form to the Medical Records Department at HealthCARE Express.

Cost for Medical Records

We charge a fee for medical records to cover the costs of copying, mailing and/or other supplies associated with your request. Upon receiving your request, we will inform you of the fee with an invoice.

1-10 pages $25.00
Additional pages $0.50 per page
Postage (rates vary based on size of medical record)
Affidavit/Notary Fee $15.00
X-Rays or Dignostic Imaging $8.00
